A single consultation reveals your status at one point in time. What determines outcomes over a lifetime is what happens between those points — and whether there is a physician paying attention.

Context compounds over time

A cardiologist who has followed you for three years understands the significance of a new finding in a way that a consultant reviewing your chart for the first time cannot. Longitudinal relationships improve the quality of clinical judgment.

Risk is dynamic, not fixed

Cardiovascular risk changes — with age, lifestyle, new diagnoses, medications, and family history. Ongoing oversight catches these changes when they are still manageable, rather than after they have progressed.

Prevention requires continuity

The evidence for preventive cardiology is clear: earlier identification and sustained management of risk factors substantially reduces cardiac events. That work requires a physician who is consistently present.

If you enroll in any membership tier within 7 days of your initial consultation, a credit is applied toward your first month. The credit is capped so Dr. Nyange is always fairly compensated for the consultation itself.

About Membership

Can I start with a single visit before committing to membership?

Yes. Individual consultations are available to any patient — virtual new patient visit ($400, 60 min) or in-person Comprehensive Cardiovascular Evaluation ($750, 60 min). If you decide to enroll in membership within 7 days of your initial visit, that fee is credited toward your first 3 months.

Can I apply my first visit fee toward membership?

Yes — if you enroll within 7 days of your initial visit, your $400 visit fee is applied in full toward Month 1 of membership. Maintenance members ($150/mo) have Month 1 fully waived. Performance members ($300/mo) have Month 1 fully waived. Executive members ($500/mo) owe $100 for Month 1. Founding Hybrid members ($625/mo) owe $225 for Month 1. The credit applies to your first monthly fee only, does not carry beyond Month 1, and cannot be applied toward the Founding Hybrid initiation fee. Regular membership rates apply from Month 2 onward.

Can I see Dr. Nyange for additional visits outside my membership?

Yes. Additional visits are billed at standard rates: virtual follow-up $295 (30 min), in-person follow-up $395 (30 min), extended longevity/cardiometabolic visit $595 (45–60 min). Performance members receive 20% off these rates. Executive and Founding Hybrid members have additional visits included.

Does membership include insurance billing?

ElinMed does not bill insurance directly. Membership fees are cash-pay. A detailed superbill is provided after each visit for potential out-of-network reimbursement through your PPO insurer. Lab and imaging orders can be submitted to your insurance carrier separately.

When does the Founding Hybrid membership open?

The Founding Hybrid tier launches with in-person care at Midtown East in Fall 2026. Virtual care for Founding Members begins immediately upon enrollment. The $2,500 initiation (which includes the in-person evaluation and advanced imaging) will be scheduled for Fall 2026 or later.
