Chronic Management

Hypertension Management

Hypertension is among the most common and most treatable cardiovascular conditions — and among the most consequential when it goes unmanaged.

What this includes

  • Comprehensive blood pressure evaluation and staging
  • Medication initiation, optimization, and adjustment
  • Home monitoring protocol and log review
  • Advanced assessment for secondary causes when indicated
  • Lifestyle and dietary guidance (DASH, sodium, exercise)
  • Target organ assessment — cardiac, renal, vascular
Most hypertension management can be handled entirely virtually. Home blood pressure monitoring is reviewed between visits with direct physician messaging.

Advanced Lipids

Cholesterol & Lipid Management

Standard cholesterol panels miss a significant portion of cardiovascular risk. Dr. Nyange goes beyond LDL — incorporating ApoB, Lp(a), inflammatory markers, and metabolic context to build a complete lipid risk picture.

What this includes

  • Full lipid panel with advanced markers — ApoB, Lp(a), hs-CRP
  • ASCVD risk calculation and 10-year risk assessment
  • Statin and non-statin therapy management
  • Lifestyle and dietary optimization
  • Residual risk assessment and treatment strategy
Lp(a) is a genetically determined lipid marker that is not reduced by statins. Testing once is recommended for all adults — and informs lifelong risk management.

Rhythm Assessment

Palpitations & Arrhythmia

Palpitations are one of the most common reasons patients seek cardiac care — and one of the most varied in terms of underlying cause. Dr. Nyange evaluates palpitations comprehensively, from benign ectopic beats to clinically significant arrhythmias requiring management.

What this includes

  • Detailed symptom history and cardiac review
  • EKG interpretation (in-person, or review of tracings sent digitally)
  • Wearable ECG review — Apple Watch, KardiaMobile
  • Ambulatory monitoring coordination (Holter, Zio patch)
  • Echocardiogram if structural cause suspected
  • AFib management including rate/rhythm control strategy
  • Anticoagulation management when indicated

Chronic Management

Heart Failure Care

Heart failure requires close, longitudinal oversight — medication titration, volume management, and regular reassessment. Dr. Nyange provides ongoing heart failure management with the continuity that this condition demands.

What this includes

  • Guideline-directed medical therapy (GDMT) initiation and optimization
  • Fluid and volume management guidance
  • Daily weight and symptom monitoring protocols
  • Echocardiographic follow-up coordination
  • Device therapy coordination (ICD, CRT) when indicated
  • Hospitalization prevention through proactive monitoring
Heart failure management through ElinMed works best as a membership arrangement, given the need for consistent oversight, direct access, and proactive medication adjustment.

Primary Focus

Preventive Cardiology

ElinMed is built around prevention as its primary mode of practice — not as a secondary consideration. Most cardiac events are preceded by years of detectable, manageable risk. The goal is to identify that risk early, understand it precisely, and act on it before it causes harm.

What this includes

  • Comprehensive cardiovascular risk assessment
  • Advanced biomarker panel — ApoB, Lp(a), hs-CRP, metabolic markers
  • Family history review and genetic risk assessment
  • Coronary artery calcium (CAC) scoring coordination
  • Advanced coronary CT imaging when clinically indicated
  • Lifestyle prescription — diet, exercise, sleep, stress
  • 10-year ASCVD risk calculation and reduction roadmap

Advanced Imaging

Advanced Coronary Imaging

Standard cardiology relies on stress tests and calcium scoring to assess coronary artery disease — tools that only detect blockage once it is already advanced. ElinMed incorporates advanced coronary CT analysis with AI-assisted plaque characterization to identify and characterize plaque years before it causes a clinical event.

Where a standard stress test only detects obstruction, coronary CT analysis reads the composition of plaque itself — stable versus vulnerable, calcified versus soft. This is the difference between reacting to disease and preventing it.

What this includes

  • Coordination of coronary CT angiography (CCTA) at a participating imaging center
  • AI-assisted plaque analysis — volume, composition, vulnerability
  • Vessel-level analysis across all major coronary arteries
  • Year-over-year progression tracking with repeat scans
  • Results reviewed directly with Dr. Nyange in a dedicated follow-up visit
Coronary imaging is included in the Founding Membership initiation and is available to all patients when clinically indicated. As heard discussed on Peter Attia, Huberman Lab, and Rhonda Patrick's platforms.

Specialized Care

Women's Heart Health

Cardiovascular disease is the leading cause of death in women — and the most underdiagnosed. Women's cardiac symptoms are frequently atypical, and the research is clear that they are more often attributed to anxiety or dismissed without adequate investigation. ElinMed brings focused attentiveness and clinical expertise to women's cardiovascular care.

What this includes

  • Sex-specific risk assessment — including hormonal status, pregnancy history
  • Evaluation of atypical symptoms including fatigue, jaw pain, back pain, dyspnea
  • Hypertensive disorders of pregnancy and preeclampsia history assessment
  • Peripartum cardiomyopathy evaluation and follow-up
  • SCAD (spontaneous coronary artery dissection) risk and management
  • Menopause-related cardiovascular risk management
If you have been told your symptoms are "just anxiety" or have had cardiac concerns dismissed, Dr. Nyange will give them the rigorous evaluation they warrant.

Expert Review

Second Opinions

Before proceeding with a significant cardiac diagnosis, procedure, or treatment plan, a second opinion from an independent expert is a reasonable and often advisable step. Dr. Nyange provides thorough, written second opinions on cardiac diagnoses, test results, medications, and surgical recommendations.

What this includes

  • Complete review of outside records, imaging, and test results
  • 60-minute virtual consultation to discuss findings and questions
  • Written summary of Dr. Nyange's assessment and recommendations
  • Alternative options and clinical context
  • Coordination with your existing care team if appropriate

Diagnostic Review

ECG & Holter Review

Digital tracings — ECGs from Apple Watch, KardiaMobile, or traditional 12-lead EKGs — can be submitted directly to Dr. Nyange for expert interpretation. Holter monitor and patch monitor reports can also be reviewed as part of ongoing care or as a standalone consultation.

What this includes

  • 12-lead EKG interpretation with written report
  • Apple Watch ECG and KardiaMobile tracing review
  • Holter monitor / Zio patch report interpretation
  • Clinical correlation and recommendations
  • Communication of findings with actionable next steps

Surgical Clearance

Preoperative Evaluation

Surgeons and anesthesiologists routinely request cardiac clearance before elective procedures. ElinMed provides comprehensive preoperative cardiovascular evaluations — currently via virtual consultation, with physical exam and on-site EKG available when in-person care opens Fall 2026. A formal clearance letter is provided for your surgical team.

What this includes

  • Review of surgical plan and anesthetic risk factors
  • Comprehensive cardiac history and risk stratification
  • Review of prior cardiac testing and imaging
  • Physical exam and 12-lead EKG — in-person (Fall 2026) or coordinated locally
  • Perioperative risk assessment per ACC/AHA guidelines
  • Formal written clearance letter for surgical team
  • Medication management guidance (anticoagulants, beta-blockers)
Same-week appointments are typically available for preoperative clearance. Please book as early as possible to allow time for any additional testing your surgeon may require.

Questions

Common Questions

Do you accept insurance?
ElinMed is a cash-pay practice and does not bill insurance directly. A detailed superbill is provided after each visit for potential out-of-network reimbursement through your PPO insurer. Labs and imaging can be ordered through your insurance separately.
Can I be seen virtually, or do I have to come in person?
ElinMed currently sees patients virtually across New York State. Most cardiology care — new patient evaluations, follow-ups, lab reviews, medication management — can be handled entirely virtually. In-person visits at Midtown East open Fall 2026 for patients who need or prefer a physical examination.
How are labs and imaging handled?
Dr. Nyange sends lab orders electronically to your preferred lab (Quest, LabCorp, or a facility near you). These can be submitted through your insurance or paid self-pay. Imaging referrals are coordinated to centers near you. All results are reviewed directly with Dr. Nyange.
What if I need a test that requires being in person?
For tests requiring in-person equipment (EKG, echocardiogram, stress test), Dr. Nyange will coordinate a referral to an appropriate facility near you until in-person care opens at Midtown East in Fall 2026. Results are reviewed by Dr. Nyange as your cardiologist of record.
How quickly can I get an appointment?
Same-week availability for virtual consultations. Book directly through the scheduling tool on the homepage. There are no months-long waitlists.
